Disability Certificate (SADAREM)
A disability certificate is an official document issued by a government medical board that certifies the type and percentage of a person's disability. It is needed to claim disability reservations in jobs and education, receive pension benefits, get a disability identity card (UDID), and access other welfare entitlements.
- Persons with physical, visual, hearing, speech, or intellectual disabilities who do not yet have a certificate - Families of children with disabilities who need a certificate for school admission or welfare schemes - Existing certificate holders whose certificate has expired and needs renewal - Persons who want to apply for the Unique Disability ID (UDID) card, which is a national ID for all disabilities
1. Visit the Telangana SADAREM portal (state's disability management system) or the national UDID portal. 2. Register with your Aadhaar number and fill in the application form with personal details and details of the disability. 3. Upload supporting documents: Aadhaar card, medical reports or prescriptions from a doctor, and a photograph. 4. Select your nearest government medical assessment centre or district hospital that conducts disability assessments. 5. After submission, you will receive an appointment date for a medical board assessment. 6. Attend the assessment with original documents. The medical board examines and records the disability type and percentage. 7. Download the disability certificate from the UDID portal once issued; a physical UDID card may also be sent by post.
